Next Steps

IF YOU ARE A PATIENT INSURED BY COMMERCIAL INSURANCE OR SEEKING SELF PAY PRICES (Non Medicaid)

We will review your insurance benefits and email over a complete verification within (48) hours. This verification will have information regarding cost share and maximum benefits as well as the terms and conditions of your plan. You will be requested to review the form, sign and submit back to our office before we can begin the process of scheduling the evaluation.

As part of the initial therapy process, we will be requiring the following information:

(Please gather all information that may be applicable to your child.  By doing so, you will expedite the process)

• A copy of your child’s IEP or other educational plan
• A referral from your PCP or specialist (if you do not have a referral, we can acquire one on your behalf). Alternatively, you can call your child’s doctor and request a referral for therapy services.  By doing so, you may expedite the process.
• A copy of any prior therapy evaluations (within the last year)
• A copy of any psychology or audiologist reports
• A copy of any ABA assessments
• Any other clinical assessment or documentation in which may be relevant

The information listed above will be requested from you during the initial on boarding process.  By gathering the information now, you will not only expedite the process, you will be assisting your child by providing a valuable picture of what is going on in your childs life.
